Alesse: Reliable Hormonal Contraception with Low-Dose Efficacy

Alesse is a combination oral contraceptive pill designed to provide effective pregnancy prevention with a lower hormonal load. It contains a balanced formulation of ethinyl estradiol and levonorgestrel, two well-studied synthetic hormones that work synergistically to inhibit ovulation, thicken cervical mucus, and alter the endometrial lining. Trusted by healthcare providers and patients for decades, Alesse offers a predictable menstrual cycle, reduced menstrual cramps, and lighter bleeding for many users. It is a suitable option for individuals seeking a convenient, reversible form of birth control with additional non-contraceptive benefits.

Features

  • Contains 20 mcg ethinyl estradiol and 0.1 mg levonorgestrel per active tablet
  • 21-day active pill regimen followed by a 7-day placebo interval
  • Monophasic formulation—each active pill has identical hormone levels
  • Small, easy-to-swallow tablets with clear packaging and day-of-week labeling
  • Manufactured under strict pharmaceutical quality control standards

Benefits

  • Highly effective at preventing pregnancy when taken correctly, with a Pearl Index of approximately 0.3–2.3 per 100 woman-years
  • May result in lighter, shorter, and less painful menstrual periods
  • Can reduce the risk of iron-deficiency anemia due to decreased menstrual blood loss
  • Offers non-contraceptive benefits such as improvement in moderate acne
  • Provides a reversible and discreet form of contraception without procedural intervention
  • May decrease the risk of ovarian cysts and ectopic pregnancy

Common use

Alesse is primarily prescribed for the prevention of pregnancy. It is also used off-label to manage menstrual-related disorders, including dysmenorrhea (painful periods), menorrhagia (heavy menstrual bleeding), and irregular cycles. Some clinicians recommend it for the treatment of moderate acne in patients who also desire contraception. It is not indicated for emergency contraception or post-coital use.

Dosage and direction

Take one tablet daily at approximately the same time each day, with or without food. Begin the first pack on either the first day of menstruation (Day-1 start) or the first Sunday after menstruation begins (Sunday start). For 21-day packs: take one active tablet daily for 21 days, then take no tablets or a placebo tablet for 7 days before starting the next pack. For 28-day packs: take active tablets for 21 days, followed by 7 inert placebo tablets. Menstruation usually occurs during the placebo week. Use a backup contraceptive method (e.g., condoms) for the first 7 days of the first cycle.

Precautions

  • Not protective against sexually transmitted infections (STIs); barrier methods are recommended for STI prevention.
  • Smoking increases the risk of serious cardiovascular side effects, especially in women over 35 years old.
  • May influence lipid metabolism, blood pressure, and glucose tolerance; periodic monitoring is advised.
  • Reduced efficacy may occur with gastrointestinal disorders (e.g., vomiting or diarrhea) or concomitant use of certain medications.
  • May affect vision or tolerance to contact lenses; consult an ophthalmologist if changes occur.

Contraindications

  • History of or current thrombophlebitis, thromboembolic disorders, or cerebrovascular accident
  • Known or suspected carcinoma of the breast or estrogen-dependent neoplasia
  • Undiagnosed abnormal genital bleeding
  • Pregnancy or suspected pregnancy
  • Severe hepatic dysfunction or liver tumors
  • Known hypersensitivity to any component of Alesse
  • Major surgery with prolonged immobilization
  • Classic migraine with focal neurological symptoms

Possible side effects

Common side effects (may improve after the first few cycles):

  • Nausea, vomiting, or abdominal cramping
  • Breast tenderness or enlargement
  • Headache
  • Changes in weight or appetite
  • Spotting or breakthrough bleeding
  • Mood changes or decreased libido

Less common but serious side effects (seek medical attention if experienced):

  • Sudden severe headache, dizziness, or faintness
  • Visual disturbances or loss of vision
  • Severe chest pain, shortness of breath, or coughing blood
  • Severe leg or arm pain or swelling

Drug interaction

Alesse may interact with the following:

  • Antibiotics (e.g., rifampin, griseofulvin)
  • Anticonvulsants (e.g., carbamazepine, phenytoin, topiramate)
  • Antifungals (e.g., griseofulvin)
  • HIV medications (e.g., ritonavir, efavirenz)
  • St. John’s Wort and other herbal supplements
  • Certain anticoagulants and thyroid medications

Concurrent use may reduce contraceptive efficacy. Always inform your healthcare provider of all medications, supplements, and herbal products you are taking.

Missed dose

If one active pill is missed (24–48 hours late): Take the missed pill as soon as remembered, and take the next pill at the regular time. No backup contraception is needed.

If two active pills are missed in Weeks 1 or 2: Take two pills on the day remembered and two pills the next day. Then resume one pill daily. Use backup contraception for 7 days.

If two active pills are missed in Week 3, or three or more pills are missed at any time: Discard the current pack and start a new pack on the same day. Use backup contraception for 7 days. Consider emergency contraception if unprotected intercourse occurred.

If placebo pills are missed, discard them and resume regular schedule; no action required.

Overdose

Serious ill effects from overdose are not expected. Symptoms may include nausea, vomiting, and withdrawal bleeding. There is no specific antidote. Symptomatic and supportive care is recommended. Contact a poison control center or healthcare provider for guidance.

Storage

Store at room temperature (15–30°C or 59–86°F). Keep in the original blister pack, protected from light and moisture. Keep out of reach of children and pets. Do not use beyond the expiration date printed on the packaging.
