Eurax: Expert-Prescribed Relief from Intense Pruritus
Eurax (crotamiton) is a topical antipruritic and scabicidal agent formulated for the symptomatic relief of severe itching and the eradication of Sarcoptes scabiei infestations. Its dual-action mechanism provides both immediate soothing effects and targeted parasitic control, making it a cornerstone in dermatological treatment protocols. This monograph provides a comprehensive, evidence-based overview for healthcare professionals to guide clinical application and patient counseling.
Features
- Active Ingredient: Crotamiton 10% w/w
- Pharmaceutical Form: Topical cream or lotion
- Mechanism of Action: Exerts antipruritic effects through local anesthesia and scabicidal activity via direct toxicity to mites
- Onset of Action: Pruritic relief often begins within minutes of application; scabicidal eradication typically requires a full treatment course
- Presentation: Available in 60g and 100g tubes
Benefits
- Provides rapid and sustained relief from debilitating pruritus associated with various dermatoses and scabies infestations.
- Eradicates scabies mites and their eggs, resolving the underlying parasitic cause of itching.
- Soothes inflamed skin, reducing the urge to scratch and minimizing further skin damage and risk of secondary bacterial infection.
- Offers a non-corticosteroid alternative for pruritus management, avoiding potential steroid-related side effects like skin atrophy.
- Simple, patient-friendly application protocol suitable for home use under medical supervision.
- Formulated to be minimally irritating for most patients, enhancing treatment adherence.
Common use
Eurax is primarily indicated for the treatment of scabies, a contagious skin infestation caused by the Sarcoptes scabiei mite. It is also extensively prescribed for the symptomatic relief of pruritus (itching) stemming from various conditions, including but not limited to: atopic dermatitis, allergic contact dermatitis, urticaria, and pruritus of unknown origin. Its use is warranted in cases where intense itching disrupts sleep, daily activities, or poses a risk for lichenification and impetiginization due to persistent scratching.
Dosage and direction
For the treatment of scabies:
- Bathing: Take a warm bath or shower to cleanse the skin and open pores. Dry thoroughly.
- Application: Apply a thin layer of Eurax cream or lotion to the entire body from the chin down, paying particular attention to areas such as finger webs, wrists, axillae, buttocks, groin, and under fingernails. Avoid application to the face, eyes, mouth, and acutely inflamed or exuding skin unless directed by a physician.
- Reapplication: A second application is recommended 24 hours after the first.
- Bathing: 48 hours after the final application, take a thorough bath to remove the medication.
- Clothing/Bedding: All clothing, bedding, and towels used within the 48 hours prior to treatment should be washed in hot water and dried on a high heat setting to prevent reinfestation.
For the relief of pruritus:
- Apply a thin layer to the affected areas and massage gently until absorbed. Repeat as needed or as directed by a physician, typically 2-3 times daily.
Precautions
- For external use only. Avoid contact with eyes, mucous membranes, and broken skin.
- Discontinue use and consult a physician if condition worsens, or if signs of a severe skin reaction (e.g., intense burning, redness, swelling) occur.
- Do not apply to extensive areas of the body for prolonged periods unless under direct medical supervision, as systemic absorption, though low, is possible.
- Use with caution in pediatric and geriatric populations, as their skin may be more sensitive or permeable.
- Do not cover the treated area with occlusive dressings unless advised by a doctor.
Contraindications
Eurax is contraindicated in patients with a known hypersensitivity to crotamiton or any of the formulation’s excipients. It should not be used on patients with a history of allergic contact dermatitis to the product.
Possible side effect
While generally well-tolerated, the following adverse reactions may occur:
- Common (≥1/100 to <1/10): Localized skin reactions including mild burning, stinging, itching, or redness at the application site. These are often transient.
- Uncommon (≥1/1,000 to <1/100): Contact allergic dermatitis, skin rash, and urticaria.
- Rare (<1/1,000): Systemic allergic reactions are extremely rare but possible, especially with application to large areas of damaged skin.
Drug interaction
No formal drug interaction studies have been conducted with topical crotamiton. Due to its localized action, systemic interactions are unlikely. However, concomitant use with other topical medications (e.g., corticosteroids, retinoids, other antipruritics) on the same area of skin is not recommended unless directed by a physician, as it may alter the absorption or effects of either product.
Missed dose
If a dose is missed for the scabies treatment regimen, apply it as soon as remembered. However, if it is almost time for the next scheduled application, do not double the dose; simply continue with the regular dosing schedule. For pruritic relief, apply the next dose at the regular time.
Overdose
Topical overdose is unlikely to cause systemic toxicity due to limited absorption. Excessive application may increase the risk of local adverse effects such as severe skin irritation, redness, and burning. In case of accidental ingestion, seek immediate medical attention or contact a Poison Control Center. Symptoms of ingestion may include nausea, vomiting, and gastrointestinal distress.
Storage
Store at room temperature (15°C to 25°C or 59°F to 77°F). Do not freeze. Keep the tube tightly closed and stored in the original carton to protect from light. Keep out of the reach and sight of children. Do not use after the expiration date printed on the packaging.
