Common use

Dosage and direction

The over-the-counter formulation is taken as one 60 mg capsule three times daily with meals. Administration should occur during the meal or up to one hour after eating. If a meal is missed or contains no fat, the dose should be omitted. The medication should not be taken more than three times daily. Treatment should be discontinued if no meaningful weight loss occurs after 12 weeks.
Precautions
Patients should be advised to follow a nutritionally balanced, reduced-calorie diet containing approximately 30% of calories from fat. Distribution of daily fat, carbohydrate, and protein intake should be evenly divided across three main meals. A multivitamin supplement containing fat-soluble vitamins (A, D, E, K) should be taken once daily at least 2 hours before or after orlistat administration. Use with caution in patients with history of hyperoxaluria or calcium oxalate nephrolithiasis. Monitor renal function in patients at risk. Not recommended for use during pregnancy or breastfeeding.
Contraindications
Orlistat is contraindicated in patients with chronic malabsorption syndrome or cholestasis. It should not be used by patients with known hypersensitivity to orlistat or any component of the formulation. Use is contraindicated during pregnancy and in organ transplant recipients receiving cyclosporine therapy. Not recommended for use in pediatric populations or patients with anorexia nervosa or bulimia.
Possible side effects

Orlistat may reduce absorption of fat-soluble vitamins (A, D, E, K) and beta-carotene. It may decrease plasma levels of amiodarone, cyclosporine, levothyroxine, antiepileptic drugs, and oral contraceptives. Patients taking these medications should separate administration by at least 2 hours. Orlistat may potentiate the anticoagulant effect of warfarin; monitor INR regularly. Concurrent use with acarbose is not recommended. May affect absorption of antiretroviral medications.
Missed dose
If a dose is missed, it should be skipped and the regular dosing schedule resumed with the next meal. Do not double the dose to make up for a missed administration. Consistent timing with meals optimizes therapeutic effect while minimizing gastrointestinal adverse events.
Overdose
There are no reports of serious adverse events from orlistat overdose. Single doses of 800 mg and multiple doses up to 400 mg three times daily for 15 days have been studied without significant adverse effects. Management should be symptomatic and supportive. In cases of significant overdose, monitor for possible gastrointestinal effects and ensure adequate hydration.
Storage
Store at room temperature between 15Β°C and 30Β°C (59Β°F and 86Β°F). Keep in the original container with the cap tightly closed to protect from moisture.
