Common use
Clarinex is primarily indicated for the relief of symptoms associated with two main conditions. First, for seasonal allergic rhinitis (hay fever) and perennial allergic rhinitis in adults and children 6 months of age and older. Symptoms treated include sneezing, rhinorrhea (runny nose), pruritus (itching) of the nose, palate, and throat, and ocular symptoms such as itchy/watery eyes. Second, for the treatment of the uncomplicated skin manifestations of chronic idiopathic urticaria (CIU), commonly known as hives of unknown cause, in patients 6 months of age and older. It is used to reduce the pruritus and number of hives.
Dosage and direction
The dosage of Clarinex is based on the patient’s age and the specific formulation prescribed. Adherence to the prescribing physician’s instructions is paramount.
- Adults and Adolescents (12 years and older): The recommended dose is 5 mg (one tablet or two 2.5 mg teaspoonfuls of syrup) once daily.
- Children (6 to 11 years): The recommended dose is 2.5 mg (one-half tablet or one 2.5 mg teaspoonful of syrup) once daily.
- Children (12 months to 5 years): The recommended dose is 1.25 mg (one 1.25 mg teaspoonful of syrup) once daily.
- Children (6 to 11 months): The recommended dose is 1 mg (one 1 mg teaspoonful of syrup) once daily.
- Orally Disintegrating Tablets (ODT): Should be placed on the tongue immediately after opening the blister. The tablet disintegrates rapidly and can be swallowed with or without water. Administration with water is not necessary.
- Clarinex may be taken with or without food.
Precautions
Before initiating treatment with Clarinex, patients should discuss their full medical history with their healthcare provider. Special consideration is needed for patients with renal impairment. For those with significantly impaired renal function, a lower starting dose may be recommended. While Clarinex is minimally metabolized by the liver, caution is advised in patients with severe hepatic impairment. Patients should be advised to engage in activities requiring mental alertness, such as driving or operating machinery, with caution until they are certain the medication does not cause drowsiness. Use during pregnancy or breastfeeding should only be undertaken if the potential benefit justifies the potential risk to the fetus or infant, as determined by a physician.
Contraindications
Clarinex is contraindicated in patients with a known hypersensitivity to desloratadine, any of its inactive ingredients, or to loratadine. Due to the structural similarity, patients who have exhibited an allergic reaction to loratadine may also be at risk for a reaction to desloratadine.
Possible side effect
Clarinex is generally well-tolerated. The most common adverse reactions in clinical trials were similar to those reported with placebo. However, like all medications, side effects can occur.
- Very Common (≥1/10): Fatigue, dry mouth, headache.
- Common (≥1/100 to <1/10): Pharyngitis, dizziness, somnolence (sleepiness), nausea, dyspepsia, myalgia.
- Uncommon (≥1/1,000 to <1/100): Hypersensitivity reactions (such as rash, pruritus, urticaria), tachycardia, palpitations, insomnia, hallucinations, seizures, hepatitis, elevated liver enzymes.
- Rare (<1/1,000): Anaphylaxis.
Patients should contact their healthcare provider if any side effect becomes severe or does not go away.
Drug interaction
Formal drug-drug interaction studies have demonstrated no significant clinical impact on the pharmacokinetics of desloratadine when co-administered with ketoconazole, erythromycin, azithromycin, fluoxetine, or cimetidine. However, due to its primarily renal excretion, the potential for interactions with other drugs that are actively secreted by the same renal tubular transport system cannot be entirely ruled out. Patients should always inform their doctor of all prescription, over-the-counter, and herbal products they are taking.
Missed dose
If a dose of Clarinex is missed, the patient should take it as soon as they remember. However, if it is almost time for the next scheduled dose, the missed dose should be skipped, and the regular dosing schedule resumed. Patients should not take a double dose to make up for a missed one.
Overdose
In the event of a suspected overdose, medical attention should be sought immediately. Symptoms of overdose may be related to an extension of the drug’s pharmacologic activity and could include drowsiness, tachycardia, and headache. Standard supportive measures are recommended. Gastric lavage may be considered shortly after ingestion. Desloratadine is not eliminated by hemodialysis.
Storage
Clarinex should be stored at room temperature, between 20°C to 25°C (68°F to 77°F), in a dry place, protected from light and moisture. The tablets should be kept in their original blister packaging until the moment of use. The syrup should be kept in its original bottle. All medications must be kept out of reach of children and pets.
