Nexium (esomeprazole magnesium) is a proton pump inhibitor (PPI) designed to provide long-lasting control of gastric acid production. It is clinically proven to treat conditions such as gastroesophageal reflux disease (GERD), erosive esophagitis, and Zollinger-Ellison syndrome. By targeting the proton pumps in the stomach lining, Nexium offers sustained symptomatic relief and promotes healing of acid-related damage. Available in both prescription and over-the-counter formulations, it represents a cornerstone in gastroenterological therapeutic management.
Features
- Contains esomeprazole magnesium as the active ingredient
- Available in delayed-release capsules (20 mg and 40 mg) and packets for oral suspension
- Designed for once-daily dosing, with or without food
- Formulated with enteric coating to protect the medication from stomach acid
- Manufactured under strict pharmaceutical quality controls
Benefits
- Provides 24-hour relief from frequent heartburn
- Reduces gastric acid secretion by inhibiting proton pumps in parietal cells
- Promotes healing of erosive esophagitis
- Helps prevent recurrence of GERD symptoms
- May be used in combination with antibiotics for Helicobacter pylori eradication
- Improves overall quality of life by minimizing discomfort and sleep disruption due to acid reflux
Common use
Nexium is indicated for the short-term treatment (4 to 8 weeks) of erosive esophagitis, symptomatic gastroesophageal reflux disease (GERD), and maintenance of healing of erosive esophagitis. It is also used for risk reduction of NSAID-associated gastric ulcers and as part of combination therapy for H. pylori eradication. Over-the-counter Nexium is approved for treating frequent heartburn (occurring two or more days per week).
Dosage and direction
For most adults, the recommended dosage is 20 mg or 40 mg once daily, taken at least one hour before a meal. The capsule should be swallowed whole; it must not be crushed, chewed, or split. For patients who have difficulty swallowing, the contents may be mixed with applesauce and consumed immediately without chewing. For oral suspension, mix the contents of the packet with water as directed and drink within 30 minutes. Dosage and duration should be individualized based on diagnosis and response.
Precautions
- Not intended for immediate relief of heartburn; may take 1 to 4 days for full effect
- Long-term use (beyond recommended duration) may be associated with increased risk of fractures, hypomagnesemia, and vitamin B12 deficiency
- Use with caution in patients with hepatic impairment
- May mask symptoms of gastric malignancy; exclude malignancy before initiating treatment
- Avoid in patients with osteoporosis or at risk for mineral deficiencies; monitor bone health and magnesium levels periodically
Contraindications
Nexium is contraindicated in patients with known hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation. Concomitant use with rilpivirine-containing products is contraindicated due to potential for reduced antiviral efficacy.
Possible side effect
Common side effects may include headache, diarrhea, nausea, abdominal pain, flatulence, constipation, and dry mouth. Less frequently, dizziness, rash, or cough may occur. Serious adverse effects (though rare) include Clostridium difficile-associated diarrhea, acute interstitial nephritis, cutaneous lupus erythematosus, and subacute cutaneous lupus erythematosus. Report any severe or persistent symptoms to a healthcare provider.
Drug interaction
Nexium may interact with drugs that require gastric acid for absorption, such as ketoconazole, iron salts, and digoxin. It can also affect metabolism of medications metabolized by CYP2C19, including warfarin, diazepam, and clopidogrel. Concomitant use with methotrexate may increase methotrexate serum levels. Consult a healthcare professional before combining with other medications.
Missed dose
If a dose is missed, take it as soon as remembered unless it is nearly time for the next dose. Do not double the dose to make up for a missed one. Resume the regular dosing schedule.
Overdose
Limited data exist on esomeprazole overdose. Symptoms may reflect exaggerated pharmacological effects, such as gastrointestinal disturbances. There is no specific antidote; provide symptomatic and supportive care. Hemodialysis is not likely to be effective due to high protein binding.
Storage
Store at room temperature (20β25Β°C or 68β77Β°F), in a dry place, away from light and moisture. Keep the bottle tightly closed and store in the original container. Do not use beyond the expiration date printed on the packaging.
