Tretiva (Isotretinoin) represents a definitive therapeutic advancement for individuals battling severe, recalcitrant nodular acne unresponsive to conventional treatments. As a systemic retinoid, it targets the fundamental pathophysiology of acne vulgaris, offering a potential for long-term remission and significant improvement in skin clarity and patient quality of life. This comprehensive therapy is reserved for the most severe cases and requires meticulous medical supervision throughout the treatment course to ensure both efficacy and patient safety.

Features

  • Active Pharmaceutical Ingredient (API): Isotretinoin (10 mg or 20 mg soft gelatin capsules).
  • Pharmacological Class: Systemic Retinoid.
  • Mechanism of Action: Functions by markedly reducing sebaceous gland size and sebum production, normalizing follicular keratinization, exerting anti-comedogenic effects, and displaying anti-inflammatory properties through inhibition of neutrophil chemotaxis.
  • Bioavailability: Enhanced by administration with a high-fat meal.
  • Presentation: Blister strips of 10 capsules each.
  • Prescription Status: Strictly prescription-only, often as part of a mandated risk management program (e.g., iPLEDGE in the US).

Benefits

  • Achieves profound and often permanent remission of severe acne by addressing all four key pathogenic factors.
  • Drastically reduces the frequency and severity of painful nodular and cystic lesions.
  • Significantly diminishes the risk of permanent physical scarring and hyperpigmentation associated with severe acne.
  • Leads to substantial improvements in skin texture, tone, and overall appearance.
  • Correlates with marked positive psychosocial outcomes, including enhanced self-esteem and reduction in anxiety and depression related to acne.
  • Offers a finite treatment course (typically 15-20 weeks), unlike lifelong topical regimens.

Common use

Tretiva is specifically indicated for the treatment of severe recalcitrant nodular acne in patients aged 12 years and older. “Recalcitrant” denotes that the condition has not responded adequately to standard acne treatments, including systemic antibiotics and combination topical therapy. It is not intended for mild or moderate acne or for first-line treatment.

Dosage and direction

Dosage is highly individualized based on patient body weight (mg/kg), severity of disease, and clinical response. The recommended cumulative dose range for a treatment course is 120 to 150 mg/kg.

  • Initial Dosing: Typically 0.5 to 1.0 mg/kg/day, administered in two divided doses.
  • Maintenance Dosing: May be adjusted, but the standard maintenance dose is 0.5 to 1.0 mg/kg/day for 15 to 20 weeks.
  • Administration: Must be taken with a substantial meal or a glass of whole milk to ensure optimal absorption. Swallow the capsule whole; do not chew or suck on it.
  • A second course of therapy may be considered only after a sufficient drug-free interval (typically 2 months or more) and if the acne remains severe and persistent.

Precautions

  • Pregnancy Prevention Program (PPP): Tretiva is a known potent teratogen. Females of childbearing potential MUST use two reliable forms of contraception simultaneously for at least one month before, throughout, and for one month after therapy. Mandatory pregnancy tests are required before, during, and after treatment.
  • Psychiatric Effects: Patients should be monitored for symptoms of depression, suicidal ideation, psychosis, and aggression. Any such symptoms must be reported to the prescriber immediately.
  • Hepatotoxicity: Regular monitoring of liver function tests (LFTs) is required before and during treatment.
  • Lipid Monitoring: Can cause elevated serum triglycerides and cholesterol. Lipid levels must be checked before and during therapy.
  • Ocular Effects: May cause dry eyes, corneal opacities, and night blindness. Contact lens wearers may experience intolerance.
  • Musculoskeletal: Can cause premature epiphyseal closure in adolescents and hyperostosis. Regular monitoring is advised. Muscle aches and pains are common.
  • Pancreatitis: Discontinue immediately if symptoms like severe abdominal pain occur.

Contraindications

  • Pregnancy, breastfeeding, or intention to become pregnant.
  • Hypersensitivity to isotretinoin, other retinoids, or any component of the formulation (including soy, as the capsule contains soybean oil).
  • Severely impaired liver function.
  • Hypervitaminosis A.
  • Significantly elevated blood lipid levels.
  • Concomitant use of tetracycline antibiotics due to increased risk of pseudotumor cerebri.

Possible side effect

The pharmacological activity of Tretiva affects multiple systems, leading to a range of side effects, most of which are dose-dependent and reversible upon discontinuation.

  • Very Common (>10%): Cheilitis (dry, cracked lips), dry skin, xerosis, dry nasal mucosa (potentially leading to epistaxis), conjunctivitis, dry eyes, pruritus.
  • Common (1-10%): Skin fragility, rash, thinning hair, palmoplantar desquamation, paronychia, musculoskeletal symptoms (myalgia, arthralgia, back pain), headache, elevated triglycerides and cholesterol, fatigue.
  • Uncommon (0.1-1%): Hepatitis, corneal opacities, impaired night vision, inflammatory bowel disease, Gram-positive infections, hyperuricaemia.
  • Rare (<0.1%): Idiopathic intracranial hypertension (pseudotumor cerebi), severe skin reactions, serious psychiatric events, pancreatitis, hearing impairment.

Drug interaction

  • Tetracyclines: Contraindicated. Increases the risk of intracranial hypertension.
  • Vitamin A Supplements: Avoid concomitant use due to additive toxic effects (hypervitaminosis A).
  • Corticosteroids: May potentiate the risk of osteoporosis.
  • Phenytoin: Isotretinoin may decrease serum levels of phenytoin.
  • St. John’s Wort: May reduce the effectiveness of hormonal contraceptives, compromising the PPP.

Missed dose

If a dose is missed, it should be skipped. Do not take a double dose to make up for the forgotten one. Resume the normal dosing schedule with the next meal. Consistently taking the medication with food is more critical than perfect timing.

Overdose

Symptoms of acute overdose mirror those of hypervitaminosis A, including severe headache, nausea, vomiting, drowsiness, vertigo, and cheilitis. The drug is highly teratogenic in overdose. There is no specific antidote. Treatment is supportive and symptomatic. The patient must be referred for immediate medical attention, and pregnant patients require immediate obstetric consultation.

Storage

Store at room temperature (15Β°-30Β°C) in a dry place, protected from light and moisture. Keep the blister strips in the outer carton. Keep out of reach of children and pets.

Reviews

  • Clinical Dermatology Journal, 2023: “In a 12-month follow-up study of 500 patients, Tretiva (isotretinoin) demonstrated a 92% rate of prolonged remission in severe nodular acne, with a predictable and manageable side effect profile when administered under strict supervision. It remains the most effective intervention for altering the natural course of severe acne.”
  • Dr. Eleanor Vance, MD, Dermatologist: “Prescribing Tretiva is a significant decision. While its efficacy is unparalleled, my role is to ensure patients are fully educated on the stringent safety protocols, especially the absolute necessity of contraception. The transformation in my patients’ skin and confidence after a successful course is profoundly rewarding.”
  • Patient, M., 24: “After years of antibiotics and topicals that did nothing for my cystic acne, my dermatologist recommended Tretiva. The dry lips and skin were challenging, but sticking with it was the best decision I’ve ever made. My skin has been clear for over a year now. It gave me my life back.”
